Birth registration

Required documents

  • Document confirming the identity of parents;
  • Completed application of parents;
  • Marriage certificate of parents;
  • Birth certificate of parents;
  • A document from a medical institution confirming the birth fact, time, and place;
  • In case of a child is born out of a medical institution, a document issued by a doctor who assisted for the birth or a doctor whom the parents applied to after birth;
  • In case of a child born dead, a document given by a doctor regarding the death;
  • In case of a child had died before the time the mother checked out of the medical institution, a document issued by the doctor confirming birth and death;
  • Regarding found children, a document confirming time, place, and condition regarding the children is found (act, protocol, reference, and a document issued by a medical institution proving the age of the child);
  • Court resolution that legally entered into force regarding the determination of the birth fact;
  • In case of the prescribed period of 1 month for the registration of birth has expired, a reference by the doctor on the health of the child.  

 Note:

In cases where the mentioned documents are issued by the official authorities of the relevant foreign country, they must be submitted either with a certificate (apostille) attached to the official documents, or legalized, and in both cases translated into Azerbaijani with the translation duly notarized.

 

 List of persons required to be present: 

  1. Parents or one of them;
  2. If parents cannot apply for a valid excuse, their relatives, neighbors, the administration of the medical institution where a child was born, or authorized persons, based on a notarized power of attorney

 

State fee:

Registration of birth and issuing primary certificates about birth are free of charge.

 

Note:

According to the Administrative Offences Code of the Republic of Azerbaijan, in case of parents do not apply for registration of birth during 1-month period without any valid excuse, the child’s parents are warned or fined an amount of 10 AZN.
